Adding intake forms

This is something you can do through your site in a few different ways, depending on what works best for you. The easiest way to do this would be to integrate an EHR platform on your Squarespace site. Almost any third-party platform can be integrated on Squarespace websites using a Code Block and a snippet of Embed Code provided by your third-party platform.

Alternatively, you could offer blank copies of new client forms on your site that clients could download and then complete and submit via email. Learn more about adding downloadable forms here.
